printed, without modification , without smears)

2.Application report

3.The applicant's original valid passport and visa

4.Any of the following work permits for foreigners

1 Foreigner Work Permit and a printed copy of the information obtained by scanning the QR code

on it

2 Affirmative Administrative Licensing Decision made by the Cultural and Tourism Department

5.Accommodation Registration Form

6.Medical Check Report within 6 months

7.Business License

8.Other required relevant documents
